UPDATE CLIENT DETAILS
Please complete this to update an existing client record that is already existing on Affinity.
When attending our office, please note the following:
you will need to see the concierge in the building lobby to register your attendance in the building;
you will not be able to access the lift and will need to contact us so that we can meet you in the lobby;
please maintain social distancing at all times;
you will have your temperature taken on arrival;
use the hand sanitiser provided at reception;
wear a face covering at all times during your appointment;
no refreshments will be offered; and
practice good hygiene at all times.
